The Pension, Hospitalization and Benefit Plan of the Electrical Industry (PHBP) is pleased to announce some exciting new improvements to your health plan.

Until now, preventive services, such as your annual well-care visit, were covered only if they were performed at the Joint Industry Board Medical Department. As of April 1, 2011, all Plan participants and their eligible dependents are able to obtain this important benefit at their own physician’s office. The annual well-care visit is covered 100% when performed at the Joint Industry Board Medical Department, and there is a $25 co-payment when it is performed at a physician’s office.

In addition, adult immunizations and prescription pre-natal vitamins are also covered.

The PHBP introduced the Connect to Health program April 1, 2011. This exciting new wellness and health management program gives you access to a variety of important health and wellness tools and support designed to make it easier to get and stay healthy—to improve your well-being and possibly reduce costs associated with your health care.

Visit www.myconnecttohealth.com to use the interactive tools, resources, information and online programs, complete your Healthways' Well-Being AssessmentTM (WBA) and access your personalized Web-based Well-Being Plan. (Note: The first time you visit www.myconnecttohealth.com you will be asked to register to gain access to the site. Scroll to the bottom of this page for instructions.)

The Connect to Health program, administered by Healthways, is in addition to the PHBP’s current benefits. Healthways and MagnaCare will coordinate for benefit eligibility purposes.

Connect to Health encourages you to be proactive regarding your health, to be educated about your health care and to take responsibility for maintaining a healthy lifestyle.

The Connect to Health program includes a smoking cessation program, a well-being assessment and health coaching for those in need of additional support, as well as many Web-based tools to help you take that first step to a healthy lifestyle—through our wellness partner, Healthways—all free to you (covered 100% by the Plan).

Use the interactive tools, resources, information and online programs at www.myconnecttohealth.com, starting April 1, 2011.This site was created by the PHBP to be your online wellness home. You can complete your Healthways' Well-Being Assessment (WBA) and access your personalized Web-based Well-Being Plan, as well as online fitness, nutrition and stress management programs.

 

Note: The first time you visit www.myconnecttohealth.com you will be asked to register to gain access to the site. Simply follow these easy steps:

  • Click on the "Not Registered? Sign Up Now!" button.
  • Enter your First and Last Name, Date of Birth, Gender, and ZIP Code. Then click on the "Next" button.
  • Complete your registration by creating a unique user ID and password, entering your e-mail address, and answering the security question.

Once you complete your registration, you will need to log in to access your personal information when you return to the site.
