Adoption of a Child?
To add a newly-adopted child to your Medical and Dental Plans, submit an Add/Delete Dependents Form (PDF, 54K) to the Joint Industry Board upon the child’s adoption or placement for adoption. In addition, you may want to consider updating your beneficiary information (PDF, 20K).
