Death?
In the event of a participant’s death, the participant’s spouse or other beneficiary must notify the Joint Industry Board and submit a certified copy of the participant’s death certificate as soon as possible. In the event of a dependent’s death, you should inform the Members’ Records Department and update your beneficiary information (PDF, 20K), if necessary.
