Birth of a Child?
To add a newborn child to your Medical and Dental Plans, submit an Add/Delete Dependents Form (PDF, 54K) and a copy of the birth certificate to the Members’ Records Department at the Joint Industry Board within 30 days of the birth of the child. In addition, you may want to consider updating your beneficiary information (PDF, 20K).
