The programs are designed to encourage all staff and participants to live healthier lifestyles and create a culture of health and wellness throughout our individual communities.

At JIB medical our greatest interest has always been preventive medicine. It is much better to stay healthy than it is trying to fight off diseases once you are already sick. Currently 95% of medical care is just reactive – something goes wrong and doctors try to fix it. More and more, people are beginning to realize how much better it is to avoid things from going wrong and how effective good preventive care can be.

Frequently Asked Questions

  • What to do if I lose my medical eligibility?

JIB Medical does not determine eligibility, but is advised by Members’ Records. The PHBP restricts access to only participants in the PHBP for all JIB Medical services. If you lose eligibility, JIB Medical has to halt your medical care here. Information from your medical record is always available to you. Removing or restoring eligibility is not up to JIB Medical. Please discuss with your Union representatives or Members Records about what you can do to be reinstated in the Members Records active participants’ lists.  You can reach Medical Records Department by calling (718)591-2000, ext. 2491.

JIB Medical is not an urgent care center, but will be available to those who lose eligibility for 30 days after becoming ineligible and will help find a new doctor if desired.

  • Can I continue to be involved in my child’s care after he/she turns 18 years old?

A person over the age of 18 is considered a legal adult and must consent to their parents continued involvement in their medical care. Consent must be documented on a HIPAA Designation Form. This form can be obtained through JIB Medical’s clerical office. If your child is intellectually or developmentally disabled, and incapable of making decisions for themselves, you must seek court approval to act as their legal guardian after your child is over the age of 18. This proof must then be provided to JIB Medical so that we can continue to involve you in your child’s medical care.

  • How can I obtain copies of my medical records?

You must complete a HIPAA Request for Access Form in order to receive copies of your medical records. This form can be obtained through JIB Medical’s clerical office.

  • What is considered a light meal prior to a non-fasting bloodwork?

Before lab work, a meal low in calorie, fat and sugar is fine. Please be sure to advise us that you ate before the lab tests were drawn, and when.

  • What is the optical/vision benefit and what does it cover?

    • You may also use a provider who belongs to the Vision Screening Optical Group. If you obtain the benefit in this way there will be no charge to you. You do not have to file a claim form. Please contact the Fund Office at (718) 591-1100 to request an optical voucher for a New Jersey panel provider. OR
    • You may have an eye examination by any other optometrist or ophthalmologist, licensed and practicing in the United States or its possessions. You may purchase eyeglasses, if necessary, from any licensed facility operating in the United States or its possessions. If you use this method, you must file a claim form with the Fund Office. The Plan will reimburse you or your optical provider according to the Plan’s fee schedule.
  • What are the fasting instructions for a fasting bloodwork?

For routine annual exam blood work, patients can come in without fasting, but it is very important to tell us if you have eaten. The food intake prior to blood tests should be very modest and not greasy or over-sugared.

Eating affects blood sugar, but if we know you have eaten, we can adjust for that. Blood draw after eating does not affect cholesterol levels, but does give us a better idea of your level of triglycerides (blood fats)

Some tests must be fasting, which means no food of any kind. If fasting, you should take lots of water and any medicines you are on.
